Semaglutide: Research Overview

The short version
Semaglutide is a synthetic peptide that copies and extends the action of GLP-1, a gut hormone released after eating that tells the pancreas to release insulin and tells the brain to feel full. It is FDA-approved for type 2 diabetes, chronic weight management, reducing cardiovascular events in people with heart disease and obesity, and, since 2025, a form of fatty liver disease. It is given as a once-weekly injection or a once-daily pill.
Of the five compounds on this reference, semaglutide has by far the largest and most rigorous evidence base: multi-thousand-person randomized trials measuring hard outcomes like heart attacks, kidney failure, and body weight, not just lab markers. That evidence base also documents a real side-effect profile, dominated by gastrointestinal symptoms, that this page reports plainly rather than downplaying.
What it is
Semaglutide is a 31-amino-acid acylated analog of human GLP-1, sharing roughly 94% sequence homology with the native hormone. Two substitutions give it its defining pharmacology: at position 8, alanine is swapped for alpha-aminoisobutyric acid to block enzymatic breakdown by DPP-4, and at position 34, lysine is swapped for arginine. A remaining lysine at position 26 carries a C18 fatty di-acid side chain, which binds reversibly to serum albumin and shields the peptide from clearance — the structural basis for once-weekly dosing rather than the roughly two-minute half-life of native GLP-1.

How it works
Semaglutide activates GLP-1 receptors throughout the body. At the pancreas it potentiates glucose-dependent insulin release and suppresses inappropriate glucagon release, meaning it works mainly when blood sugar is already high. It also slows gastric emptying, which extends fullness after a meal and contributes to the nausea some people experience.
Its weight-loss effect is thought to be primarily central: semaglutide reaches appetite-regulating circuits in the hypothalamus and brainstem, activating neurons that signal fullness and quieting neurons that drive hunger. This reduces overall food intake and, according to research-community reports, the background mental preoccupation with food some describe as "food noise" — without lowering how much energy the body burns.
What the research shows
Weight management (STEP 1). In 1,961 adults with overweight or obesity and no diabetes, once-weekly semaglutide 2.4 mg produced a mean body-weight change of -14.9% at 68 weeks versus -2.4% with placebo [11].
Cardiovascular outcomes (SELECT). In 17,604 adults with established cardiovascular disease and overweight or obesity but no diabetes, semaglutide reduced the composite of cardiovascular death, non-fatal heart attack, or non-fatal stroke versus placebo (hazard ratio 0.80, a 20% relative reduction) [10].
Kidney outcomes (FLOW). In 3,533 adults with type 2 diabetes and chronic kidney disease, semaglutide 1.0 mg reduced major kidney-disease events versus placebo (hazard ratio 0.76, a 24% lower risk) [9].
Head-to-head against tirzepatide. In the SURMOUNT-5 trial of 751 adults with obesity, tirzepatide produced greater mean weight loss than semaglutide at 72 weeks (-20.2% versus -13.7%, a statistically significant difference) — the clearest available evidence that semaglutide, while effective, is no longer the strongest agent in its own drug class [8].
Safety profile. A dedicated safety review concludes semaglutide carries an overall favorable risk-benefit profile in type 2 diabetes, with mostly mild-to-moderate, transient gastrointestinal effects (nausea in roughly a third of patients), an increased risk of gallbladder disease, and pancreatic- and thyroid-cancer signals for which the review says definitive conclusions cannot yet be drawn given low incidence [12].
Reported effects, cautions & safety
Reported in research and patient communities — anecdotal, not clinical evidence, and never a dose: The most consistently described benefit is a sharp drop in appetite and food preoccupation, often within the first week or two, along with reduced cravings for sugar and fried food and, for many, steady weight loss over months. Some people using semaglutide for diabetes describe markedly better blood-sugar readings, and a number report an unexpected drop in interest in drinking alcohol. On the downside, nausea is the single most reported complaint, sometimes with vomiting, typically worst in the first weeks and after dose increases; distinctive sulfur-smelling burps, constipation or diarrhea, acid reflux, early fatigue, and occasional headaches or dizziness are also frequently described. A smaller group reports hair shedding or a more hollow-looking face months in, generally attributed to the pace of weight loss rather than the medication itself.
Cited safety cautions:
- Gastrointestinal intolerance, especially during dose escalation, is the dominant clinical adverse effect and the leading cause of discontinuation [12].
- A personal or family history of medullary thyroid carcinoma or MEN-2 is a contraindication, based on a boxed warning drawn from rodent thyroid-tumor data; a dedicated review notes this signal has not been established in humans [12].
- Acute pancreatitis is a class warning; treatment is conventionally stopped if it is suspected [12].
- Gallbladder and biliary disease risk is increased, attributed largely to the pace and scale of weight loss rather than a direct toxic effect [12].
- Diabetic retinopathy can worsen early when blood sugar is corrected rapidly in people who already have retinopathy, a signal seen in a large controlled trial [12].
- Beyond the cited review, the broader literature documents further mechanism-based cautions worth knowing: some of the weight lost is lean muscle mass rather than fat, weight tends to return substantially after stopping the medication, hair shedding has appeared as a pharmacovigilance signal tied to rapid weight loss, and semaglutide is contraindicated in pregnancy, requiring a documented washout period before a planned conception.
